JOB ROLE:
- Lead and inspire a new team of care professionals, including Care Assistants and Senior Care Assistants.
- Provide supervision, guidance, and support to ensure our team delivers top-quality care.
- Develop and implement personalised care plans in consultation with clients, their families, and healthcare professionals.
- Regularly monitor and review care plans to ensure they meet clients' evolving needs.
- Conduct comprehensive assessments of clients' care needs and develop tailored care plans.
- Ensure care plans are consistently reviewed and updated.
- Ensure all care services comply with relevant regulations, policies, and procedures.
- Conduct regular audits and quality checks to maintain high standards of care.
- Build and nurture positive relationships with clients, their families, and healthcare professionals.
- Act as the main point of contact, providing information, support, and reassurance.
- Offer ongoing training, support, and development opportunities to enhance staff skills and knowledge.
- Ensure all team members are competent and confident in delivering care services.
- Maintain accurate and up-to-date records of care plans, assessments, and other relevant information.
- Utilise appropriate systems and technology for record keeping and reporting.
- Ensure compliance with health and safety regulations, including risk assessments and infection control.
- Address any health and safety concerns or incidents promptly.
- Flexibility to support our of hours On-Call service
REQUIREMENTS:
- Experience as a Care Coordinator or Care Manager with relevant qualifications in social care, nursing, or a related field.
- NVQ Level 5 is desirable, but candidates with lower-level qualifications are encouraged to apply, with full support for further learning and education provided.
- Proven experience in managing or coordinating care services, preferably in a homecare or community setting.
- Strong knowledge of relevant regulations, policies, and procedures in social care.
- Excellent communication and interpersonal skills, with the ability to build positive relationships.
- Ability to lead, manage, and motivate a team effectively.
- Exceptional organisational and time management skills, with the ability to prioritise tasks.
- Strong problem-solving skills and the ability to make sound decisions.
- Proficiency in using computer systems and technology for record keeping and reporting. Experience of PASS System is desirable
- A full current driving licence and access to own vehicle is essential for this role.
