Care Coordinator – Part Time

Heritage Healthcare Trafford and Cheshire enjoy a respected reputation as the care provider of choice for many local families. Our person-centred service allows individuals with varying healthcare needs, to remain at home for longer, with the support of our dedicated Home Care professionals.

Due to continued growth, we are looking for a Care Coordinator on a part time basis (approx. 10 hours per week) to support our team in ensuring the delivery of outstanding care and service.  This can be a busy and challenging role, within a busy office environment, focusing on delivering a working schedule for our clients and carers every week.  The ability to prioritise and remain calm under pressure is key.


The role will include;

  • Ensuring work schedules are prepared considering travel time, holidays, training, and last-minute cancellations
  • Working closely with the team and owners in co-ordinating all care services
  • Managing daily communications within the office
  • Developing good relationships with clients and carers
  • Adding client and carer information onto to the scheduling system and to maintain all records as per company policy
  • Dealing promptly and appropriately with any queries received or concerns raised
  • Ensuring compliance and quality assurance is met in line with company procedures and CQC regulations
  • Providing cover in other business areas as and when required

  • Ability to remain positive and professional when working under pressure
  • An extremely organised individual, with excellent planning and prioritising ability
  • Excellent literacy with great attention to detail
  • Excellent communication skills, both on the phone and in person with previous experience of delivering outstanding customer service
  • Personal qualities – we want caring, trustworthy, compassionate, reliable, and flexible people
  • Computer literacy – IT experience is essential and previous use of care software would be an advantage
